- A statement under penalty of perjury that you have a good-faith belief the material was removed by mistake or misidentification.
- Your consent to jurisdiction of the Federal District Court for the judicial district where your address is located, or if outside the U.S., the Federal District Court for New York County, New York, and that you will accept service of process from the notifier.

- Repeat Infringers
- We may terminate or block repeat infringers where appropriate.
- Misuse
- Knowingly submitting false notices or counter-notices may result in liability.
